In order to get credit for your course(s), upon completion of your online class(es), you must return the original notarized affidavit to The Professional Development Institute, Attention: Andre Costas, 2730 University Blvd, Ste. 200, Wheaton, Maryland 20902. (An original notarized affidavit is required. Do not fax the affidavit. Either mail or bring the original notarized affidavit to Andre at the above address.
